Job Function: Responsible for the overall management of central intake including oversight of personnel, resources and program components necessary for effective services. The Liaison is an Agency leader and is also responsible for demonstration of core values and interpreting the mission of the Agency in all activities. Provides oversight and implementation of a community-wide service system of resources that ensures families are linked to needed resources and services and specific needs of individuals and families are identified and addressed effectively; Strengthens care coordination and systems integration across sectors; Helps to improve communication between families and providers; Coordinates a plan for outreach and education of central intake services to families, social service agencies, health care providers and other community stakeholders; Convenes a community advisory board; Effectively collaborates with partners to eliminate duplication of effort and services, and maximize utilization of available resources; Maintains a standardized data tracking system and ensures quality objectives are met and activities are implemented; Participates in designated community forums and advisory boards/councils; Maintains and updates the web-based resource directory to include formal and informal resources; Identifies new resources and participates in community outreach events.

Necessary Skills and Abilities
• Must demonstrate initiative, creativity, be dependable, reliable and work well independently.
• Must be able to exercise independent thinking and good judgment under all circumstances.
• Must be able to analyze work, set goals, develop plans and utilize time effectively and efficiently.
• Must regularly be able to see, speak and hear. Frequently required to stand, sit, walk, bend, use hands and arms and must occasionally lift up to 20 pounds.
• Must be sensitive, flexible and responsive to gender, race, ethnicity, socio-economic status, religion, age, sexual orientation or any other special needs as reflected in the ability to communicate with staff and/or clients.
• Assume an active role in agency events.

To qualify for this job you need to have the below minimum requirements and experience:
• Must have Bachelor degree in social work or related field.
• Must have and maintain a valid driver's license, use of an insured vehicle and an acceptable driving record.
